Switch Kit Forms
Below you will find the forms needed to help you switch to U.P. Connection Federal Credit Union. It is fast, convenient and easy to do. Just input your information into the forms and print. Remember, before you send out the forms to stop by any of our convenient locations and open your new account.
1. Switching to U.P. Connection Form
This form will allow you to notify and transfer funds from your old account(s) to your new U.P. Connection account(s). If you are switching to a U.P. Connection checking account, make sure to leave enough money in your old account(s) to cover any outstanding checks.
2. Direct Deposit Request Form
Direct Deposit makes life easier. If you currently use Direct Deposit, complete this form and give it to your payroll office at work. If you would like to utilize the convenience and safety of Direct Deposit, check with your payroll office at work to see if this service is offered. If so, complete the form and give it to your payroll office. If you receive pay from the Government or receive Social Security click here to make the change online or download the change form.
3. Electronic Payment Change Request Form
If you utilize electronic payments that are automatically made from your existing checking account, please complete this form to continue payments through your new U.P. Connection Checking Account. Some payments may include your mortgage, insurance, credit cards, utilities, loans or investments.
